BAXI diverter valves

so, just bumped into cust/pal of mine with duotec calls me last monday to a report of hot tap running and rads heating up!
called in tuesday with a new brass diverter cartridge, followed the instructions, backed off the flow connector to avoid hydralic damage etc.
after the job, it worked as it should until the thurdsay, now the same problem has reoccured!

any ideas why this has happened?
